Signing up to play for the Dolphins is a 3 step process.

Step 1 – Membership Form

Please download and complete the Club Membership Form – 456Kb
Email membership form, a copy of your child’s birth certificate, and a recent photograph
(photo to be in jpg format 600px x 600px and no more than 1Mb)

Step 2 – BokSmart

Please download and complete the Bok Smart Participation Screening Questionaire – 663Kb
Email to chairman@ballitodolphinsrugby.co.za

Step 3 – Payment

Ensure the required payment has been made to:
